Instructions for KYC (Read first)Read the below instructions carefully to get all the supporting documents ready, before starting KYC Data Entry and Document Upload
The KYC form inputs and documents are to be provided by filling the forms separately for each Insured Person for whom Life Cover is needed Create a ZIP file with all the below mentioned applicable documents and then upload via "Choose File" button above 1. PAN Card 2. Address Proof (Aadhaar / Drvg Lic / Util Bill / Voter ID / Govt Letter / Property Tax / Tenancy Agreement - India and Intn'l) 3. Passport - Front & Back with Visa Stamp (NRIs only) 4. National or Resident ID (NRIs only) 4. Empty (Unsigned) Cheque (of Bank a/c to be used for making the Policy Premium - Should be INR Currency Bank account Cheque - Premium payment from Foreign Currency Bank accounts are not accepted - Cheque leaf should have name printed) 5. Passbook (or) Bank Statement - last 1 month (if cheque leaf does not have name printed - Passbook or Bank Statement should mandatorily display Account Holder Name, Account Number, Bank Name, Branch Name, Branch address (with Pin Code), NEFT/IFSC Code and MICR Code) 6. Married Women's Property Act (MWP) - Insurance Policies where Nominee's are incorporated as per MWP Act cannot be changed later. Linking your Life Insurance Policy under MWP Act ensures that the Insurance Claim money cannot be attached or claimed by any other party other than those mentioned as Nominee(s) (Wife and / or multiple children along with % Allocation - Pls select MWP option and mention list of Nominees along with their relationship to you and their Date of Birth and whether Minor or not) (Files should be of JPG (or) PNG or PDF Format only - Other formats cannot be processed. Store all the above document files in a folder, select all files and upload (or) Zip the entire folder and upload via "Upload" button in left side, after entering the details in form fields, and finally press "Submit" button) |
